46 deaths in 78 boss pulls (0.59 per pull) · 26% to avoidable damage · 8 to abilities without a note
Mythic-only: each Imbibe also drops Malignant Tumors/Totems across the raid's half of the room (Hexweaver counts 24). They have 99% damage reduction (Hardened Tumor) until a Plague Wave passes over them; any still standing just after the NEXT Imbibe detonate as Malignance, a heavy stacking raid hit - one is survivable, two or more usually are not.
Several players (3 Normal / 4 Heroic / 5 Mythic per wowcarry) are debuffed for ~6 sec, pulsing damage to anyone within 4.5 yards; on expiry each fires four Plague Waves in the cardinal directions that travel to the walls. On Mythic these waves are also the tool that strips Malignant Tumors.
Shadow DoT with a screen effect, from the blood side of the Sentinels (the Bloodvenom family around Bloodvenom Injection). The screen effect is the tell that it is on you.
One half of the Sentinels' combined 'Mark of Acid / Mark of Blood' entry, split out because deaths are recorded under this name alone. Standing within 40 yards of the Acid golem (Breath of Ula'tek) applies its stacking mark roughly every 5 seconds, lasting 40 seconds; both groups and tanks trade sides at every Vitriolic Stasis so neither mark becomes unsustainable.
Fire AoE that is the dispel consequence of Exploding Infection: when the infection is dispelled or resolves badly, it detonates as Caustic Explosion.
Nature damage: the return lines sent back toward the Breath when Toxic Droplets are destroyed on Heroic. The first pass describes these lines under Toxic Droplets; this is the spell that damages you when one connects.
If any living venom slime reaches the Malignant Cavity at the room's center, it detonates: heavy raid Nature damage plus a stacking 30-second DoT. On Mythic the Burst leaves a long (one-minute) stacking DoT, raising the price of every leak.

Fire slimes radiate a burning aura while alive (Burning Presence) and can be gripped/slowed/CC'd. On death a Burning Venom explodes (Caustic Surge), dealing raid damage and applying a raid Fire DoT, so kills must be staggered rather than simultaneous.

Shadow bolt with a snare, cast during the Vashnik fight.
Nature damage attached to the Sentinels' poison profile. It appears in damage taken but is not a separately journaled cast in the first pass, so treat it as part of the council's constant Nature pressure.
When a Restless Amani dies it deals Plague damage to the raid and leaves a DoT; the corpses left behind matter for the intermission. On Heroic, corpses leave a Vessel of Awakening and can be re-raised if not burned.
Kith'ix unleashes a near-lethal raid Shadow shockwave that also strips Light's Embrace. Every player must first pick up a Light's Embrace mote (which reduces Shadow damage taken by 90% for up to 8 sec); an immunity or major defensive is a backup for one missed mote, not a strategy.
Shadow raid damage from the ascended explorer's presence (the Mor'zahi side of The Lost Explorers). Background rot for the fight's second half profile.
One half of the Sentinels' combined 'Mark of Acid / Mark of Blood' entry, split out because deaths are recorded under this name alone. Standing within 40 yards of the Blood golem (Blood of Ula'tek) applies its stacking mark roughly every 5 seconds, lasting 40 seconds; both groups and tanks trade sides at every Vitriolic Stasis so neither mark becomes unsustainable.
Every Nullshard hit (or standing in a gate) applies a Nullify stack. At the lethal threshold the player dies outright: 5 stacks on Heroic (PlayHub reports 6 on Normal / 5 on Heroic) and only 3 stacks on Mythic. Hexweaver death reports show the stack count at death, making this the most diagnosable death on the boss.
Mythic-only. Eight players gain Protovenom circles and a ticking Plague DoT that only clears when two circle-carriers touch each other. If a carrier touches a player WITHOUT the debuff, it triggers a Protovenom Eruption blast with knockback and the debuff stays.
Triggered when a Restless Amani reaches the Soulcoil Well or a player dies in it: Nek'zali gains 5 energy and the whole raid takes a Shadow hit plus a stacking Shadow DoT. On Heroic each Rite also applies Ritual Burn, increasing damage taken from later Rites by 15% for 60 sec, stacking.
Shadow AoE burst. Distinct from the listed Stygian Infection: this is the burst damage spell that lands from the Stygian side of the fight.
The Breath sprays Toxic Droplets in front of itself; droplets left standing eventually explode, while stepping on one destroys it at the cost of Nature damage to the soaker. On Heroic, destroyed droplets send Living Venom lines back toward the Breath that must also be dodged.
A marked player erupts after 8 seconds, dealing Shadow damage split among everyone within 7.5 yards and applying stacking Clinging Murk. On Heroic, soakers' Murk expiry also drops Blood Venom pools.
